Join a pioneering long-term care team at Eddy Village Green in Cohoes, part of St. Peter’s Health Partners and Trinity Health. We are currently seeking a PT Night RN, 7p-7a 2x a week with an every other weekend. EVG is a 192-bed skilled nursing facility designed around the Green House model, offering care in 16 self-contained, 12-bed homes that prioritize elder autonomy, comfort, and community. We welcome newly licensed nurses and offer a comprehensive orientation, flexible scheduling, and a supportive, team-driven environment. Key Responsibilities: Deliver skilled nursing care using the nursing process to assess, diagnose, plan, implement, and evaluate resident needs. Conduct admission assessments and health histories. Collaborate with interdisciplinary teams to develop and implement individualized care plans. Educate residents and families about care plans and health conditions. Monitor and respond to psychological and psychosocial risks. Maintain accurate documentation and communicate effectively across shifts and departments. Support the Green House model by fostering autonomy, dignity, and meaningful relationships with residents. Requirements: Minimum Qualifications: Current New York State RN license Associate’s Degree in Nursing Preferred Qualifications: Experience in long-term care or skilled nursing Strong communication and conflict resolution skills High level of engagement and teamwork Schedule & Compensation: Shifts Available: Rotating, including 7pm–7am Pay Range: $33.10 – $45.75 Location: Cohoes, NY – Easy commute from Albany, Latham, Troy, and Clifton Park Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
