About The Position

We’re seeking five full-time Registered Nurses (RNs) to join our Dublin, Ohio team! As a Home Health RN, you’ll provide one-on-one skilled nursing care in patients’ homes, coordinate with physicians and care teams, and ensure top-quality outcomes for every patient.

Requirements

• Active RN license in Ohio
• 2+ years of home health nursing experience
• Skilled in managing diverse home health diagnoses
• Strong clinical assessment and critical thinking skills
• Valid driver’s license and reliable transportation

Nice To Haves

• BSN preferred (not required)

Responsibilities

• Conduct initial and ongoing assessments — including Start of Care (SOC), Recertifications, Resumptions, and Discharges
• Develop, implement, and evaluate individualized plans of care
• Administer medications, wound care, and treatments
• Educate patients and families on care routines, medications, and disease management
• Collaborate closely with physicians, therapists, and the interdisciplinary team
• Document care accurately in WellSky EMR (or similar)
• Ensure compliance with Medicare, Medicaid, and agency standards
• Participate in weekend rotation as needed
