About The Position

ComForCare Home Care in Baltimore County is hiring Registered Nurses to conduct in-home assessments and provide ongoing client care and case management. We have immediate openings with flexible hours and days. Position Overview: We are seeking a skilled Registered Nurse to provide clinical oversight, perform initial and ongoing assessments, and develop personalized care plans for clients receiving services in their homes. Requirements

  • Current Maryland Registered Nurse (RN) license or mutli-state licensure.
  • CPR/First Aid Certification
  • Ability to work evening and weekend shifts, as needed
  • Reliable transportation for home visits.

Nice To Haves

  • Experience in Home Health, Hospice, and/or Geriatric Nursing preferred
  • Strong assessment and clinical decision-making skills
  • Familiarity with infection control and patient safety procedures

Responsibilities

  • Conduct comprehensive in-home assessments and reassessments
  • Create and revise individualized care plans based on clinical findings
  • Provide medication administration and skilled nursing interventions
  • Monitor and document changes in client health and status
  • Communicate regularly with clients, caregivers, and care teams
  • Initiate referrals to appropriate healthcare or community resources
  • Maintain timely, accurate documentation in accordance with regulations
  • Participate and facilitate caregiver in-service programs
  • Supervision of CNA's, GNA's, HHA's
  • Client assessments.
  • Administrative support.
