ComForCare Home Care in Baltimore County is hiring Registered Nurses to conduct in-home assessments and provide ongoing client care and case management. We have immediate openings with flexible hours and days. We are seeking a skilled Registered Nurse to provide clinical oversight, perform initial and ongoing assessments, and develop personalized care plans for clients receiving services in their homes. Requirements
- Current Maryland Registered Nurse (RN) license or mutli-state licensure
- CPR/First Aid Certification
- Reliable transportation for home visits
- Familiarity with infection control and patient safety procedures
Nice To Haves
- Experience in Home Health, Hospice, and/or Geriatric Nursing preferred
- Strong assessment and clinical decision-making skills
- Ability to work evening and weekend shifts, as needed
Responsibilities
- Conduct comprehensive in-home assessments and reassessments
- Create and revise individualized care plans based on clinical findings
- Provide medication administration and skilled nursing interventions
- Monitor and document changes in client health and status
- Communicate regularly with clients, caregivers, and care teams
- Initiate referrals to appropriate healthcare or community resources
- Maintain timely, accurate documentation in accordance with regulations
- Participate and facilitate caregiver in-service programs
