About The Position

Suncrest Hospice aims to provide exceptional hospice care by increasing staff-to-patient ratios, thereby increasing clinical visits while lowering clinician caseloads. As a Community Health Accreditation Partner (CHAP) certified hospice, Suncrest seeks individuals committed to high-quality patient and family care. The Registered Nurse (RN) will be responsible for providing care and services to a group of patients, visiting them in various residences including private homes, Skilled Nursing Facilities, and assisted living facilities. This role is a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources. The Registered Nurse (RN) will function in a Case Management, Admissions, or Resource capacity.

Requirements

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
  • Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
  • Ability to work closely with patients on a 1-1 level while receiving strong support from a patient care team
  • Ability to work independently on a day-to-day basis, managing activities such as phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations
  • Possess time management skills to manage visits, documentation, and meetings (e.g., scheduled visits, required meetings, responding to urgent needs by prioritizing and triaging multiple concerns)
  • Ability to learn an electronic medical records system
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

Responsibilities

  • Provide care and services for a group of patients
  • Visit patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities
  • Contribute to the interdisciplinary team by providing expertise regarding psych-social assessments, interventions, and knowledge of community sources
  • Work in either a Case Management, Admissions, or Resource role
  • Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership within a team environment
  • Work closely with patients on a 1-1 level
  • Work independently on a day-to-day basis, managing phone, email, and written communication
  • Attend meetings with a variety of healthcare professionals
  • Maintain an understanding of hospice rules and regulations
  • Manage visits, documentation, and meetings, including scheduling visits, attending required meetings, and responding to urgent needs by prioritizing and triaging multiple concerns
