About The Position

The Registered Nurse (RN) provides professional nursing care to residents in accordance with individualized care plans, recognized nursing techniques, and established standards of practice within the scope defined by the Minnesota Board of Nursing. The RN functions in alignment with and in support of the mission, goals, and objectives of Mission Nursing Home and the Nursing Department. This position reports to the Director of Nursing and requires a strong commitment to ethical, moral, and legal responsibilities, with conduct that consistently reflects the facility’s mission and values.

Requirements

  • Graduate of an accredited nursing program.
  • Current active license as Registered Nurse in the state of Minnesota.
  • Must be able to pass a State of Minnesota Department of Human Services criminal background study, as well as a United States Office of the Inspector General Exclusion search.
  • CPR Certification is required.
  • Ability to communicate effectively in both verbal and written formats.

Responsibilities

  • Maintains accurate unit narcotic and controlled substance count records per facility policy and procedure.
  • Documents resident care and clinical information in the medical record in accordance with facility policies.
  • Uses supplies and equipment appropriately and responsibly.
  • Assists with reviewing and updating resident care plans under the direction of licensed leadership.
  • Communicates and documents all changes in resident conditions promptly and accurately.
  • Provides input and participates in resident care conferences.
  • Communicates pertinent unit activities, concerns, or personnel issues to appropriate leadership.
  • Participates in shift-to-shift handoff reporting in accordance with facility policy.
  • Communicates effectively with interdisciplinary team members and other departments.
  • Admits and orients new residents and families and completes admission documentation, as assigned.
  • Assists Nurse Assistants/Resident Assistants (NARs) with bedside care when needed.
  • Performs and documents resident weights and vital signs as warranted.
  • Administers medications and treatments as prescribed by the physician and per facility policy.
  • Orders medications from the pharmacy according to established procedures.
  • Coordinates resident discharges and transfers, completes required documentation, and arranges transportation or escort as needed.
  • Assists with resident safety needs in accordance with the facility’s Safe Patient Handling policies.
  • Implements and follows appropriate isolation and infection prevention techniques.
  • Collaborates with HUCs, Care Coordinators, or Nurse Managers to schedule resident appointments for medical, dental, diagnostic, or therapeutic services.
  • Keeps families and legal guardians informed of resident condition and needs in accordance with the plan of care.
  • Notifies physicians of unusual or abnormal resident conditions and documents accordingly.
  • Accurately receives, transcribes, and implements physician orders in MatrixCare per policy.
  • Performs resident assessments, exams, and treatments following facility procedures.
  • Accompanies physicians during rounds as needed.
  • Participates in required continuing education, including in-service training programs.
  • Maintains required continuing education units (CEUs) for licensure and renewal.
  • Assists with staff performance evaluations as assigned by the RN Nurse Manager or Director of Nursing.
  • Treats residents with dignity, respect, and compassion at all times.
  • Protects the organization’s values by maintaining confidentiality of resident information.
  • Immediately reports suspected abuse, neglect, or exploitation according to facility policy and regulations.
  • Follows standard precautions, facility safety policies, and OSHA regulations.
  • Attends all required in-services, competencies, and staff meetings.
  • Reports vulnerable adult concerns per facility policy and regulatory requirements.
  • Demonstrates knowledge of fire and disaster procedures and participates in drills.
  • Reports equipment malfunctions to designated personnel and adheres to Lockout/Tagout policies.
  • Participates in Unit Quality and Safety meetings as directed.
  • Utilizes technical resources including (but not limited to) two-way communication devices, electronic medical records (EMAR), email, web-based education platforms, and other technology.
  • Performs other duties as assigned.
