About The Position

This position delivers patient care utilizing the nursing process, which includes data collection, assessment, identification and prioritization of patient problems/needs, and evaluation of nursing care outcomes. The role involves coordinating, providing, and directing patient teaching activities, as well as coordinating care provided by other health team members. In collaboration with the interdisciplinary care team, the nurse conducts initial and ongoing patient assessments, analyzes data, creates and implements treatment plans, and evaluates their effectiveness. Responsibilities also include administering medications safely according to state and Bon Secours Mercy Health policies, serving as a point of contact for patient care coordination across hospital departments, and advocating for patient safety through participation in quality improvement initiatives. The nurse is expected to incorporate the professional practice model into care delivery.

Responsibilities

  • Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care.
  • Coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members.
  • In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness.
  • Administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures.
  • Serves as point of contact for patients' care coordination throughout hospital departments.
  • Acts as a patient safety advocate by participating in ongoing quality improvement in the department.
  • Incorporates the professional practice model into care delivery.
