About The Position
The WOCN provides expert care and coordination for patients requiring wound, ostomy, and continence management in the home setting. This role supports high-quality patient outcomes through advanced clinical practice, staff education, and collaboration with the interdisciplinary care team. This position will cover Western Nassau and Queens.
Requirements
- Bachelor’s Degree in Nursing (BSN) required
- Current New York State Registered Nurse (RN) license
- Minimum 5 years of community health nursing experience
- Strong clinical expertise in wound, ostomy, and continence care
- WOCN certification required
- Excellent communication, organizational, and problem-solving skills
Responsibilities
- Assess, plan, implement, and evaluate patient care in the home setting
- Provide specialized wound, ostomy, and continence treatments
- Monitor patient progress and adjust care plans as needed
- Serve as a clinical resource and mentor to nursing staff
- Supervise Home Health Aides and support care team performance
- Ensure accurate documentation and compliance with regulatory standards
- Collaborate with physicians and healthcare providers to ensure continuity of care
- Participate in quality improvement and patient outcome initiatives
