GENERAL STATEMENT OF DUTIES The Registered Nurse (RN) – Triage & Urgent Care provides timely clinical assessment, triage, and nursing care for patients presenting with acute and urgent health needs. This role supports safe patient flow, rapid access to care, and high-quality, patient-centered services in a fast-paced urgent care and same-day care environment. The RN collaborates closely with providers and the care team to ensure safe, efficient, and compassionate care delivery.

Requirements

  • Current and unrestricted California Registered Nurse (RN) license.
  • Current BLS certification (ACLS preferred for urgent care).
  • Minimum 3 years of RN experience.
  • Strong clinical assessment and critical-thinking skills.
  • Proficiency with electronic health records (EHR).

Nice To Haves

  • Experience in urgent care, emergency department, or nurse triage.
  • Experience in FQHC, community health, or rural health settings.
  • Familiarity with standardized triage protocols (e.g., Schmitt-Thompson).
  • Bilingual skills preferred (Spanish or other languages).

Responsibilities

  • Clinical Triage and Assessment Perform in-person and telephone triage using approved, evidence-based protocols.
  • Assess patient symptoms, vital signs, acuity, and risk factors.
  • Prioritize patients appropriately and escalate care as indicated.
  • Urgent Care Nursing Care Provide direct nursing care to urgent care patients within RN scope of practice and standing orders.
  • Administer medications, injections, IV fluids, and immunizations as ordered.
  • Perform wound care, splinting, EKGs, point-of-care testing, and other clinical procedures.
  • Assist providers with urgent and minor procedures.
  • Patient Flow and Care Coordination Support efficient patient throughput and clinic flow.
  • Collaborate with providers, MAs, CHWs and front office staff to manage patient volume.
  • Coordinate referrals to higher levels of care, including emergency departments and specialty services.
  • Provide patient education regarding follow-up care, warning signs, and self-management.
  • Telephone and Electronic Triage Respond to nurse advice calls, patient portal messages, and urgent clinical inquiries.
  • Apply standardized triage guidelines and document all encounters accurately.
  • Provide clear, culturally appropriate patient education and guidance.
  • Documentation and Compliance Document assessments, interventions, and patient communications timely and accurately in the electronic health record (EHR).
  • Ensure compliance with organizational policies, FQHC/HRSA requirements, and FTCA standards.
  • Participate in quality improvement, patient safety, and risk management activities.
  • Infection Control and Safety Follow infection prevention and control standards.
  • Maintain a clean, safe clinical environment.
  • Participate in emergency preparedness and response activities.
  • Team Collaboration and Professional Conduct Serve as a clinical resource to MAs, LVNs, and support staff.
  • Demonstrate professionalism, teamwork, and patient-centered communication.
  • Participate in staff meetings, training, and ongoing professional development.
