This job functions as the liaison and communicator with the patient, caregivers, healthcare providers, and multi-disciplinary team members as well as post-acute care and third party payers. Discusses alternative care options with patient / caregivers as well as the multi-disciplinary team and assist with discharge planning needs. Facilitates, in collaboration with the multi-disciplinary team, movement along the health care continuum to ensure quality; cost–effective outcomes are achieved.

Requirements

• Registered nurse diploma.
• Current RN license in state of practice.
• Basic Life Support (BLS.) through the American Heart Association.
• CPR Healthcare Provider Certification.
• 2 years of related experience.
• Proficiency in using computers, software, and web-based applications.
• Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
• Good organizational skills.
• Good time management skills.
• Conflict resolution skills and ability to demonstrate good judgement and decision making in performing tasks.

Nice To Haves

• Associate's or bachelor's degree in nursing.
• Certification in Case Management (CCM).
• Health Care Management (CPHM).

Responsibilities

• Manages various aspects of the planning process in anticipation of the patient’s movement through the healthcare system.
• Utilizes care managements policies and procedures including application of evidenced based criteria and level of care and continued appropriateness.
• Manages the care of assigned patients through the healthcare system based on the patient’s individual needs.
• Prepares, executes, and reinforces post-discharge care plan.
• Performs other related duties as required.
