Requirements

  • Active Registered Nurse license in the states that services will be provided.
  • Minimum 2 years’ experience in home health care or hospice setting
  • Experience working in the home health care industry or familiarity with home health care patients
  • Knowledge of relevant regulations and guidelines, such as HIPAA
  • Ability to work flexible hours
  • Strong attention to detail
  • Excellent written and verbal communication skills
  • Ability to work independently and as part of a team
  • Excellent organizational and time management skills to meet deadlines and handle multiple tasks simultaneously
  • Must pass a criminal background check

  • Bachelor’s degree preferred

Responsibilities

  • Assesses and chart observations of the patient’s condition at each visit.
  • Performs accurate and complete assessments within defined time frames.
  • Prioritizes and documents patient problems based on assessed needs/problems in collaboration with the patient, family, and health care team.
  • Completes evaluation tasks, including reviewing medication and vital signs.
  • Administers physician-prescribed medication.
  • Evaluates, updates and revises plan of care to facilitate achievement of planned and expected outcomes.
  • Provides education to patients and families on proper home health care procedures and strategies.
  • Coordinates with Physicians, Physical Therapists, and other individuals in the patient’s care plan.
  • Maintains confidentiality of all office and client records per HIPAA and Privacy Act regulations.
  • Performs other duties as assigned.
