Requirements

Current, unrestricted RN license in South Dakota
BLS/CPR certification
Minimum 2 years of nursing experience

Proficiency with EHR systems and clinical documentation

Responsibilities

Provide direct nursing care across the lifespan in a primary care setting
Perform clinical tasks including vital signs, medication administration, specimen collection, and CLIA-waived testing
Assist providers during exams and procedures
Maintain accurate documentation in the EHR
Follow all safety, infection control, and HIPAA standards
Deliver individualized education on chronic conditions such as diabetes, hypertension, asthma, and preventive care
Use culturally and linguistically appropriate teaching methods
Support patient self-management through motivational interviewing and goal setting
Coordinate care with providers and interdisciplinary team members
Identify and help address barriers to care (transportation, medication access, health literacy)
Refer patients to community resources and support services
Provide guidance and support to LPNs, MAs, and clinic staff as needed
Participate in team huddles, quality improvement initiatives, and case reviews
Assist with community outreach events, wellness fairs, and health education programs
