This Registered Nurse (RN) position in the Patient Command Center focuses on patient coordination and throughput. The role involves continuous assessment, planning, implementation, and evaluation of patient needs to facilitate proactive management of admissions, discharges, and transfers. Key responsibilities include reviewing potential admissions (from ED, direct, procedural, or inter-facility transfers) to ensure patients are placed at the most appropriate level of care based on medical necessity criteria and evidence-based practice. The RN collaborates with the healthcare team, including physicians and transferring facilities, and assists in identifying alternatives to acute care when appropriate. The role also entails maintaining accurate patient and bed management data for trending and planning, and participating in efforts to reduce risk and improve patient safety. A significant part of the position involves facilitating and coordinating patient assignments to the most suitable beds, maintaining an overview of hospital capacity, anticipating bed needs, and collaborating with various departments (ED, surgery, PACU, cath lab, Nursing) to optimize patient throughput. The RN will utilize resources and infection control guidelines for bed placement decisions, provide oversight to bed management support staff, and actively round on units to manage discharges, admissions, and patient placement. This role requires the ability to compel nursing units to accept new patients, explain clinical reasoning, and demonstrate diverse clinical knowledge. The RN will also assess patient levels of care, identify code beds, communicate bed flow status to leadership, and manage an electronic bed board. Furthermore, the RN is responsible for educating healthcare team members on admission criteria and patient placement guidelines, ensuring appropriate hospital utilization in compliance with regulations and accreditation standards. The position involves active participation in performance improvement activities, including documenting bed management program activities, utilizing evidence-based practice, and collaborating with leaders to enhance operational efficiency and patient outcomes.
