The Command Center Registered Nurse is responsible for executing all aspects of bed management and transfer center operations in a safe and timely manner, in accordance with standard operating procedures and regulatory requirements. The scope is inclusive of patient access, placement, transfers and bed management. This role uses clinical knowledge, critical thinking and communication skills to support the Command Center in providing a reliable, high-quality service that allows referrals, transfers, and other healthcare settings to be accepted in an efficient and timely manner. The nurse assesses, directs and coordinates transfers and admissions from physician offices, hospitals, and other healthcare settings to the most appropriate accepting service, facility, and level of care. They facilitate patient placement by assessing patient diagnosis, acuity and location, applicable risk factors and unit criteria / bed availability. The position involves collaboration with facility departments, medical staff, nursing staff, and leadership to promote appropriate, timely patient placement so that patient care is not delayed, and also with facility staff to identify and communicate bed capacity issues. The Command Center Registered Nurse leverages capacity dashboards and other real-time indicators to predict future patient placement pressure points and participates in the execution of surge-capacity protocols; and initiating interventions, within scope of practice, to accelerate patient flow or other mitigating barriers to optimal throughput. They assist with all aspects of patient logistics to include pre-registration, transportation, and bed planning; collect unit specific data, and information needed to access and place patients appropriately. The role adheres to regulatory compliance with all Command Center operations, including Stark, EMTALA, and any other pertinent to Call Center activities, and maintains current knowledge of regulatory requirements impacting operations of the Command Center. The nurse provides timely and accurate transfer and admission documentation, works collaboratively with providers and key stakeholders in the development and maintenance of transfer and admission pathways for patients, and delegates as needed to the Transfer Center Tech, Patient Placement Specialist, and EMS Liaison. They promote the mission, vision, and values of Intermountain Health, and abide by service behavior standards. Other duties may be assigned, and the nurse may be required to be placed on-call during a regularly scheduled shift.
