About Abernathy Home Care Abernathy Home Care has proudly served the Boston North community for over 30 years, specializing in ASAP (Aging Services Access Point) community-based care. Our services focus on supporting seniors and adults in their homes through state-funded programs, working closely with ASAP case managers to ensure safe, compliant, and high-quality care. This position is centered around ASAP consumer care, requiring strong knowledge of community-based services, compliance, and supervisory oversight. We are seeking an experienced RN or LPN for a full-time, field-based role. This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management. You will be working independently in the field while collaborating closely with case managers, families, and our internal team.
Requirements
- Active RN or LPN license in Massachusetts
- Minimum 1 year of home care experience REQUIRED
- ASAP experience strongly preferred
- Must be comfortable working independently in the field
- Reliable transportation and willingness to travel daily
- Strong assessment and documentation skills
Nice To Haves
- Bilingual Spanish preferred, but not required
Responsibilities
- Complete in-home supervisory visits for HHA/PCA caregivers
- Conduct client assessments and reassessments
- Ensure compliance with ASAP program requirements
- Communicate with ASAP case managers and families
- Monitor client conditions and report changes
- Maintain accurate and timely documentation
