About The Position

Under the direction of the Director of Nursing, the RN is responsible for the planning and coordination of direct resident care services. Serves as a core member of the interdisciplinary client care team. Has the necessary authority to assess, plan, evaluate and teach in patient situations. Carries out administrative responsibilities as delegated by Director of Nursing. Performs a focused nursing assessment including gathering and recording assessment data, observing, monitoring, and reporting signs, symptoms, and changes in a patient's condition in an ongoing manner to the RN or supervising health care provider. Is accountable for all patients assigned in accordance with established policies and procedures and within the scope of practice. Demonstrates the knowledge and skills necessary to provide care for the population served in the department.

Requirements

  • Licensed Practical Nurse (LPN) - Board of Nursing An active license in the state of practice Upon Hire
  • Must work 72 hours in a 6-week schedule (equates to 624 hours annually, not to exceed 1248 hours in a 12-month period)
  • Must work 6 weekend shifts (Friday, Saturday and/or Sunday) per year
  • Shifts are 12-hour shifts (6 a.m.-6:30 p.m. and 6 p.m.-6:30 a.m.)
  • One year of RN/LPN experience required upon hire.
  • Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
  • 3-5 years experience
  • The individual must be able to work the hours specified.
  • To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.

Responsibilities

  • Performs professional nursing care to residents, according to needs and is responsible for direct supervision of nursing activities of facility.
  • Conducts daily rounds and communicates to appropriate team members during the shift.
  • Contacts providers, establishes good working relationship and ongoing information exchange.
  • Administers routine medications, documents and make decisions about the use of prn medications.
  • Obtains specimens for lab tests as ordered.
  • Consistently & thoroughly completes documentation in a timely manner, including documentation of all medications and treatments administered and participates in the development of interdisciplinary plan of care.
  • Participates in interdisciplinary team meetings.
  • Initiates, participates and coordinates additional meetings as necessary to address crisis matters and provide support for team members.
  • Maintains established departmental policies and procedures, objectives, quality improvement, environmental safety and infection control standards.
  • Initiates resident referrals, transfers and discharges.
  • Ensures that assigned CNAs/CMAs staff understand each residents’ plan of care.
  • Assists in training and education of nursing and nursing support staff, participates in QAPI activities.
  • Maintains supplies and equipment in adequate numbers and working order following infection control policies.
  • Provides resident and family education by direct instruction, utilization of interdisciplinary team members and appropriate community resources.
