About The Position

Registered Nurse (RN) Navigator-Oncology is responsible for the coordination of patient care based on a designated tumor-site cancer population(s). This position will function autonomously and in collaboration with multiple health care providers as a patient advocate, educator, and consultant. RN Navigator- Oncology will focus on achieving quality clinical outcomes by effectively coordinating care for patients throughout the disease continuum and across multiple settings by utilizing resources effectively and efficiently. The duties of this position are performed in a manner, which promotes a multidisciplinary team concept. The patient care process (assessment, planning, implementation, and evaluation) is used for patients across the tumor-site continuum. The RN Navigator- Oncology supports and implements patient safety and other safety practices as appropriate. Provides patient and family centered care. Age-appropriate care is provided to patients and families. This position maintains professional competencies by attending educational programs, participates in professional organizations, or other appropriate activities.

Requirements

  • 3 years minimum clinical experience in Oncology
  • Active and Current MN RN Licensure
  • BLS
  • Experience and skills with patient education
  • Experience working with multidisciplinary teams

Responsibilities

  • Demonstrate understanding and knowledge of pathophysiology of specific disease processes.
  • Understand oncology treatment modalities and rationale including surgery, chemotherapy, biotherapy, radiation therapy, symptom management, survivor care, and surveillance.
  • Utilize current clinical guidelines to assist in the development and implementation of a treatment plan (i.e. NCCN guidelines).
  • Work with providers and patient to manage physical, mental, psychosocial, and/or spiritual well-being.
  • Assess current health status to address health promotion needs, functional status, development, and lifestyle issues to maximize health outcomes.
  • Collaborate in the development of treatment plan to meet patient needs and optimize disease prognosis/outcomes.
  • Apply teaching-coaching philosophy when establishing patient relationship:
  • Assessment of patient understanding of plan of care;
  • Assist patient with understanding medical terminology;
  • Utilize effective patient communication skills, including active listening.
  • Provide continuity of care throughout the entire cancer care experience from pre-diagnosis through survivorship, palliative/hospice care and end of life.
  • Develops and/or recommends patient educational resources regarding diagnosis, disease for tumor-site populations.
  • Determines implications and availability of resources at local, regional, and national levels in order to support the most inclusive and reasonable plan and continuum of care.
  • Assists in the coordination, implementation, evaluation, and modification of the overall plan for care through an interdisciplinary and collaborative team process to ensure timely, comprehensive coordination of services.
  • Communicate among provider and physician practices to build relationships.
  • Participate in Oncology specific committees.
  • Assess patient ability to navigate through the healthcare system and identify potential barriers (such as continuity, coordination of care and communication with health care team).
  • Guide patient through the healthcare system including appropriate support referrals i.e. social work, dietitian, and insurance advocate.
  • Navigate orders, ensuring that the patient has appropriate referrals to surgeons, genetic counselors, radiation oncology, medical oncology, chemotherapy, imaging, labs, etc.
