Requirements

Active RN license (Virginia)
Home care or case management experience preferred
Strong communication and relationship-building skills
Comfortable making calls and scheduling appointments
Experience with social media platforms and basic online marketing
Organized, self-motivated, and professional

Responsibilities

Clinical Duties:
Conduct in-home client assessments
Develop and review care plans
Coordinate with caregivers and families
Ensure compliance with state and agency regulations
Marketing & Outreach:
Make outbound calls to referral sources (hospitals, rehab centers, assisted living communities, physician offices)
Set up and attend appointments with potential referral partners
Build and maintain strong professional relationships
Represent the agency at networking events
Online & Social Media Management:
Manage agency social media platforms (Facebook, LinkedIn, etc.)
Create and post engaging content
Respond to online inquiries and leads
Assist with online reputation management and community engagement
