About The Position

FirstLight Home Care of St. Paul provides compassionate, high-quality in-home care for seniors and adults who wish to remain safely and independently at home. We are committed to clinical excellence, regulatory compliance, and strong communication with clients, families, and caregivers. We are seeking a Part-Time Registered Nurse (RN) to lead client intake, clinical compliance, caregiver competency oversight, and ongoing client management. This role also provides clinical phone support when caregivers or clients need guidance. This position offers guaranteed minimum weekly hours(+8) and a predictable schedule. This is not a PRN or bedside nursing role.

Requirements

  • Active Minnesota RN license (required)
  • Minimum 3 year of nursing experience
  • Strong assessment, documentation, and clinical judgment skills
  • Comfortable conducting intakes and clinical evaluations independently
  • Reliable transportation and ability to travel locally

Nice To Haves

  • Home care, hospice, or case management experience
  • Familiarity with Minnesota home care regulations
  • Experience supervising or training caregivers

Responsibilities

  • Client Intake & Assessments
  • Conduct initial and ongoing client intake assessments (in-home or virtual)
  • Evaluate client needs, risks, and service appropriateness
  • Develop, review, and update individualized care plans
  • Compliance & Clinical Oversight
  • Ensure compliance with Minnesota home care regulations and agency policies
  • Support clinical documentation standards and audit readiness
  • Participate in surveys, audits, and quality improvement activities
  • Caregiver Competency & Training
  • Conduct caregiver competency evaluations and skills assessments
  • Review and sign off on required competencies and training documentation
  • Provide clinical guidance and coaching to caregivers and CNAs
  • Client Management
  • Monitor client status and ongoing care quality
  • Address care-related concerns from clients and families
  • Collaborate with care coordinators and management on care adjustments
  • Clinical Phone Support
  • Provide clinical phone support to caregivers and clients as needed
  • Assist with urgent care questions, changes in condition, or guidance
  • Escalate issues appropriately and document clinical follow-ups
