Requirements

  • Current RN license (unencumbered) in the state of practice; CPR certification required.
  • Two years of nursing experience preferred; hospice experience strongly preferred due to role autonomy and on-call expectations.
  • Strong clinical judgment with the ability to provide compassionate end-of-life care across homes and facilities.
  • Emotionally steady, empathetic, patient-centered, and able to remain calm under pressure.
  • Excellent communication, follow-through, and the ability to support families in crisis.
  • Adaptability, integrity, and comfort working independently while staying connected to a highly supportive team.
  • Valid driver’s license, auto insurance, and reliable transportation for home visits.

Responsibilities

  • Complete initial and ongoing assessments and develop individualized hospice plans of care in accordance with physician orders and regulatory timelines.
  • Provide skilled nursing services including pain and symptom management, crisis intervention, and urgent patient/caregiver support.
  • Document clinical care accurately and timely to ensure compliance with Medicare/Medicaid Hospice Conditions of Participation.
  • Coordinate closely with the interdisciplinary group (IDG), participate in care conferences, and ensure patients and families remain informed, supported, and empowered.
  • Supervise and support LPNs and HHAs through required supervisory visits and delegated task accountability.
  • Provide education to patients and families on the hospice philosophy, end-of-life care, symptom management, safety, and patient rights.
