About The Position

Registered Nurse (RN) – Hospice Location: Santa Clara County Pay: $55.00 – $65.00 per hour Job Type: Full Time, Part Time, Per Diem The Sequoia Difference At Sequoia Hospice , we’re passionate about delivering exceptional, individualized care. We’re growing and looking for an experienced Registered Nurse (RN) to join our Hospice Care team! What You’ll Do As a Hospice RN , you will plan, organize, and direct hospice care while educating patients and families. You’ll serve as a case manager and ensure quality, compassionate care in the home setting.

Requirements

• Active CA RN License
• Minimum 2 years of nursing experience (hospice or home health experience preferred)
• Strong clinical, communication, and problem-solving skills
• Current CPR certification
• Valid driver’s license, insured vehicle in good condition
• Ability to work independently and manage priorities

Responsibilities

• Complete initial and ongoing patient and family assessments
• Develop and revise individualized hospice care plans
• Provide professional nursing care using the nursing process
• Administer medications and treatments as prescribed
• Educate and counsel patients and families on hospice care
• Coordinate care for your assigned caseload
