Registered Nurse (RN) – Hospice & Home Health (Dual Role)
About The Position
Interim HealthCare of Montgomery County is seeking a flexible and experienced Registered Nurse (RN) to support both our Hospice and Home Health services in a dual-role capacity. This position is ideal for an RN who is confident in clinical assessments, recertifications, and end-of-life care , and is looking for a role that offers variety, autonomy, and meaningful patient impact . Requirements
- Active Registered Nurse (RN) license in Tennessee
- Minimum 1 year of nursing experience (home health or hospice preferred)
- Experience with OASIS documentation strongly preferred
- Strong assessment and clinical decision-making skills
- Ability to work independently in a field-based environment
- Excellent communication and patient/family education skills
- Ability to travel to patient homes within service area
- Ability to perform patient assessments, including mobility assistance as needed

Responsibilities
- Perform hospice admissions and ongoing nursing visits
- Provide pain and symptom management in accordance with plan of care
- Support patients and families with education, emotional support, and end-of-life care
- Participate in IDG (Interdisciplinary Group) meetings
- Ensure compliance with hospice documentation and regulatory standards
- Complete Start of Care (SOC), Recertifications, and Reassessments
- Perform comprehensive clinical assessments and OASIS documentation
- Develop and update plans of care in coordination with physicians
- Ensure timely and accurate documentation for compliance and billing
- Collaborate with therapy and support staff to coordinate patient care
