Why Nurses Apply to Elara Caring • 1:1 patient care • Flexible schedules and autonomy in your day • Supportive clinical leadership • Collaborative care teams • Competitive compensation • Tuition reimbursement for full time staff • Continuing education opportunities for all employees • Medical, dental, and vision insurance • 401(k) with employer match • Paid time off and paid holidays • Family and pet bereavement leave • Pet insurance What You Will Do • Manage and coordinate your own patient caseload as a Home Health RN • Complete patient assessments and reassessments • Develop, implement, and update individualized care plans • Perform start of care visits and routine follow up visits • Educate patients and families on treatment plans and health management • Communicate changes in patient condition with physicians and the clinical team • Collaborate with an interdisciplinary team to improve patient outcomes • Utilize electronic documentation systems such as HomeCare HomeBase What You Need • Current unrestricted Registered Nurse RN license • At least 1 year of experience in a clinical care setting as a nurse • Ability to travel within the branch coverage area • Valid driver’s license and reliable transportation • Ability to frequently lift, push, pull, and support up to 50 pounds • Associate or Bachelor’s degree in Nursing preferred • Previous Home Health RN or Hospice experience preferred • Experience with HomeCare HomeBase preferred Why Home Health Nurses Love This Role You manage your own day.
