About The Position

The Home Health Registered Nurse performs a comprehensive patient assessment and develops an individualized patient plan of care with collaboration of an interdisciplinary team and physician approval. Under the direction of and in accordance with the physician's orders administers skilled nursing care to patients in their place of residence. Functions as a case manager and collaborates with the interdisciplinary team to meet patient outcomes and goals. As a leader of the healthcare team, provides professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. Demonstrates performance consistent with the PIH Health vision of Patients First. Requirements

  • Establish positive and professional relationships with patients, families, staff, and physicians.
  • Demonstrate quality nursing clinical practice while meeting time-sensitive deadlines.
  • Knowledge and ability to apply the nursing process while developing an individualized patient plan of care.
  • Able to demonstrate clinical nursing expertise in the establishment of peripheral intravenous lines and administration of intravenous therapies.
  • Computer skills and ability to navigate an electronic health record.
