Requirements

• Current, unrestricted Registered Nurse (RN) license
• Minimum 1 year of clinical nursing experience
• Ability to travel within the branch coverage area
• Reliable vehicle, valid driver’s license, and current auto insurance
• Ability to frequently lift, push, pull, or support up to 50 pounds

Nice To Haves

• Associate or Bachelor’s degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

Responsibilities

• Manage and coordinate your own patient caseload
• Complete initial and ongoing patient assessments
• Develop, implement, and update individualized care plans
• Perform start-of-care visits and routine follow-up visits
• Communicate patient condition changes with physicians and care team
• Maintain accurate documentation and HIPAA compliance
• Support patients and families throughout their care journey
