Job Description: Home Health Registered Nurse (RN) Full-Time | Elkhart | $7,500 Sign On Bonus! Care that meets patients where they are. A career that meets you where you want to be. Requirements
- Active, unrestricted RN license
- At least 1 year of nursing experience in a clinical setting
- Associate’s or Bachelor’s Degree in Nursing preferred
- Home Health or Hospice experience preferred
- Experience with HomeCare HomeBase a plus
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to travel within the service area (approximately 50%)
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Nice To Haves
- Home Health or Hospice experience preferred
- Experience with HomeCare HomeBase a plus
Responsibilities
- Completing start-of-care, routine, and follow-up nursing visits
- Performing comprehensive patient assessments
- Developing and updating individualized plans of care
- Monitoring patient progress and identifying changes in condition
- Communicating with physicians, clinical leaders, and care teams
- Documenting care accurately and timely in the EMR
- Ensuring compliance with state, federal, and agency guidelines
- Maintaining patient privacy and HIPAA compliance
