About The Position

This position performs a variety of integrated functions, intuitively, using the nursing process while applying a broad nursing knowledge in a caring, concerned manner. The role involves regularly assessing patient’s/client’s needs and providing skilled nursing services as prescribed by the physician authorized plan of care and accepted standards of nursing practice. It also includes providing direct resident care, such as medication administration, treatments, responding to call lights, assisting with transfers, and alarms. The nurse evaluates the effectiveness of the treatment plan, making suggestions or participating in updates and revisions in collaboration with other team members to improve patient care. Processing physician/provider orders accurately and timely, and following up on medication changes and order clarifications are also key. Additional duties include assessing patients for physical, emotional, psych/social, and family support, organizing and coordinating all aspects of patient care, demonstrating proficiency in nursing skills, evaluating patient progress, and communicating effectively. The role requires competency in Microsoft® Windows, educating patients and families, consulting and collaborating with the healthcare team, and providing teaching, support, and coordination of healthcare plans through the acute phase of illness and focusing on continuum of care post-discharge.

Requirements

  • BLS: Basic Life Support (within 1 month of hire or transfer)
  • ACLS: Advanced Cardiac Life Support (within 12 months of hire or transfer)
  • RN: Registered Nurse (MI)
  • DIMC: Dysrhythmia Interpretation Man (Telemetry Competency through testing or Telemetry Monitoring & Management class with passing score within 12 months of hire or transfer)
  • FINGERPRIN: Fingerprinting (Required upon hire or transfer)
  • TEAM ADVANCED: Techniques for Effective Aggression Management (within 6 months of hire or transfer, or 24 months if in this position on or before 12/31/2023)
  • Associates Degree in Nursing (NRG)
  • Minimum of two (2) years nursing experience.

Responsibilities

  • Demonstrates proficiency (according to hospital policy and procedure) in nursing skills to carry out the patient's plan of care.
  • Evaluates patient progress by correlating patient information with nursing knowledge to determine if goals are being met according to the patient's plan of care.
  • Communicates interactions, both verbal and written, that clearly and positively accomplish desired patient care as a collaborative effort.
  • Demonstrate competency in Microsoft® Windows.
  • Educates patients and families (including students from Community Colleges and Universities).
  • Consults and collaborates with members of the healthcare team in planning and implementing total patient care for individuals.
  • Provides teaching, support and coordination of healthcare plans for patient and family through the acute phase of illness, and with a focus on the continuum of care in the home and community post discharge, as needed.
