About The Position

Select Home Health is seeking a skilled and compassionate Registered Nurse (RN) – Float to provide high-quality, in-home nursing care. This role supports multiple patients and care teams as needed, ensuring continuity of care while promoting patient independence, safety, and overall well-being. The RN – Float will deliver personalized clinical services in accordance with physician orders, agency policies, and regulatory guidelines, while adapting to varying schedules and coverage needs.

Requirements

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license in the state of Indiana (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.

Responsibilities

• Perform patient assessments and ongoing evaluations.
• Implement physician-ordered plans of care and complete required documentation (OASIS).
• Provide in-home nursing care for multiple patients per day.
• Educate patients and caregivers on medications, disease processes, and in-home care.
• Coordinate patient services and supervise LPNs/HHAs as needed.
• Communicate changes in patient condition to physicians and care teams.
• Maintain compliance with regulations and agency policies.
