About The Position

This position provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization. The role involves implementing and monitoring patient care plans, recording and communicating patient conditions, and serving as a primary coordinator for all disciplines to ensure well-coordinated patient care. Key duties include noting and carrying out physician and nursing orders, and assessing and coordinating patient's discharge planning needs with the healthcare team. The nursing process is central to this role, encompassing a systematic approach to patient care. This includes initial, re-assessment, and focused assessments to collect and analyze physiological, psychological, sociocultural, spiritual, economic, and life-style data. The nurse identifies patient problems based on clinical judgment of actual or potential health conditions. Planning involves setting measurable short- and long-range goals in collaboration with the patient and family, documented in a care plan. Implementation covers ensuring patient safety, medication administration, provision of ordered treatments, patient/family education, and comprehensive documentation. Continuous evaluation of patient status and care effectiveness is required, with modifications to the care plan as needed. The role also involves care coordination, delegation, supervision, and prioritization of tasks from admission to discharge.

Requirements

  • Graduate from an accredited School of Nursing
  • Basic Life Support (BLS) - American Heart Association
  • Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days

Responsibilities

  • Implements and monitors patient care plans
  • Monitors, records, and communicates patient condition as appropriate
  • Serves as a primary coordinator of all disciplines for well-coordinated patient care
  • Notes and carries out physician and nursing orders
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team
  • Uses a systematic, dynamic way to collect and analyze data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and life-style factors (Initial assessment, Re-assessment, Focused assessment based on patient condition, setting and population)
  • Applies clinical judgment about the patient’s response to actual or potential health conditions or needs (Problem Identification)
  • Sets measurable and achievable short- and long-range goals in collaboration with the patient and family, and writes them in the patient’s care plan (Establishing daily goals with the patient and family, Formulating a plan of care, Transition or Discharge planning)
  • Implements nursing care according to the care plan, ensuring continuity of care and documenting in the patient’s record (Ensures patient safety, Medication administration, Provision of ordered treatments, Patient/Family education, Provision of nursing care, Documentation in healthcare record)
  • Continuously evaluates both the patient’s status and the effectiveness of the nursing care, and modifies the care plan as needed (Evaluation)
  • Initiates actions with patients, families, and/or members of their healthcare team to manage and correct the sequence, timing, and/or effectiveness of patient care from admission to discharge (Care Coordination)
