This position provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization. The role involves implementing and monitoring patient care plans, recording and communicating patient conditions, and serving as a primary coordinator for all disciplines to ensure well-coordinated patient care. Key duties include noting and carrying out physician and nursing orders, and assessing and coordinating patient's discharge planning needs with the healthcare team. The nursing process is central to this role, encompassing a systematic approach to patient care. This includes initial, re-assessment, and focused assessments to collect and analyze physiological, psychological, sociocultural, spiritual, economic, and life-style data. The nurse identifies patient problems based on clinical judgment of actual or potential health conditions. Planning involves setting measurable short- and long-range goals in collaboration with the patient and family, documented in a care plan. Implementation covers ensuring patient safety, medication administration, provision of ordered treatments, patient/family education, and comprehensive documentation. Continuous evaluation of patient status and care effectiveness is required, with modifications to the care plan as needed. The role also involves care coordination, delegation, supervision, and prioritization of tasks from admission to discharge.
