About The Position

The Perioperative Ambulatory Care Coordination Program supports patients and families through the full pre operative journey by ensuring they are medically ready, well informed, and connected to the right resources. The team conducts thorough pre anesthetic assessments, coordinates care across surgical and specialty services, manages time sensitive testing and documentation, and promotes a smooth, safe day of surgery experience.

Requirements

• A Bachelor of Science in Nursing
• 3 years of Pediatric RN experience in Perioperative or Surgical Inpatient programs
• Strong communication, critical thinking, and organizational skills in addition to proficient multi-tasking abilities
• Strong technical knowledge of EMR (preferably EPIC).
• MA RN license

Responsibilities

• Collaborating with all interdisciplinary team members including nurses, nurse practitioners, anesthesiologists and perioperative team for day surgery patients.
• Assisting in the coordination of patient care services within other disciplines of the healthcare system
• Reviewing patients' medical/surgical history and outside records in the EMR to identify potential perioperative risk factors.
• Performing telephone history/assessment to include review of systems, medication list, patient education, and implementing perioperative plan.
• Working (3) 10 hour shifts Mon-Fri.
