About The Position
The Patient Navigator/Registered Nurse - Cancer Care role guides patients through the care continuum, understanding the clinical pathways established and directing patients to healthcare services within the organization, at outside facilities and within the community.
Requirements
- Bachelor's Degree in Nursing required.
- Current applicable state(s) license as a Registered Professional Nurse required. Certifications specific to department policies may be required.
- 1+ years of experience is required.
- Experience working with patients and families in area of specialty preferred.
- Experience in community outreach preferred.
