Virtually tour one of the state-of-the-art units at Banner-University Medical Center Tucson. In the Registered Nurse Stroke Patient Navigator position, you will play an integral role with Banner University Medical Center Tucson’s stroke program. You will be working closely with the Neuro team and helping stroke patients through the healthcare process from diagnosis through treatment. In addition to working with the Neuro team, you will serve as a navigator between physicians, nursing, and bedside staff across multiple units and work closely with our stroke coordinator and stroke abstractor. This is a full time (40 hours/week), in-person/face to face salaried, day shift position where great interpersonal and communications skills and the ability to work autonomously are of the utmost importance. No weekends or holidays. POSITION SUMMARY This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
