About The Position

The PACT RN is a skilled nurse who is able to work proficiently with some level of supervision to optimize patient care, patient safety and operational efficiency for patients. The focus of the individual's performance encompasses a combination of clinical, administrative, education and research. The PACT RN must demonstrate performance that impacts more than one individual and patient care assignment. The Southern Arizona VA Health Care System is seeking Registered Nurse's for (PACT) in the Nursing, Ambulatory Care Service. The incumbent uses the nursing process and evidence-based practice to collaborate with the Team (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal, and community -based services involved in providing care to the patient) in developing the patient-driven care plan for life.

Responsibilities

  • The Incumbent is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs.
  • The PACT RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's personalized health plan/goals.
  • The PACT RN demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development, and practice.
  • The incumbent possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting to facilitate the effective care of patients.
  • Within the framework of the nursing process and through highly developed clinical expertise, the RN is able to assess, monitor and treat patient responses to life-threatening health problems.
  • The Incumbent is responsible for collaboration with administrative and clinical staff in actively managing and planning for actual and potential needs of patient population groups.
  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components.
  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
  • Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
