About The Position

ESSENTIAL FUNCTIONS Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. Documents patient care given. Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Works as an advocate for the physical and emotional well-being of the patient. All Surgical Services RN Requirements Utilizes initiative; strives to maintain steady level of productivity; self motivated; manages activity and time well. Actively participates in unit/hospital committees. Participates in performance improvement activities. Makes suggestions, gathers data and assists with analysis. Demonstrates knowledge of Core Measures and consistently ensures measures are met and correctly documented for the perioperative patient Educates the patient and family regarding pain management. Follows the five medication rights to reduce the potential for medication errors. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Collaborates with the patient, significant others, and health care providers in providing patient care. Demonstrates knowledge of Malignant Hyperthermia including location of cart and treatment and care of the patient in crisis. Develops a prioritized holistic plan of care on a comprehensive assessment, desired outcomes and current knowledge. Assesses the patient’s physical, psychological, social, spiritual, educational, cultural needs and discharge planning needs to determine appropriate actions. Assures that medical orders are entered correctly, communicated appropriately and carried out accurately and in a timely manner. Delivers nursing care according to the standards of care defined within the Hospital Policies and Procedures. Acquires and maintains current knowledge in nursing practice Supports and promotes excellence in customer/guest relations. Supports the philosophy, objectives and goals of the Department(s) and assesses areas of personal and professional growth. Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group. Demonstrates ability to perform timely and accurate patient assessments on assigned patients within established time frames. Consults other departments, as needed and as appropriate, to provide for an interdisciplinary approach to the patient’s needs Evaluates the patient’s and family response to care and teaching and revises the plan of care as needed. Appropriately responds to the patient in an emergency or physically distressful situation and communicates with physicians and change in patient status in a timely manner. Follows hospital policy for safe medication administration in the operating room. Effectively communicates and assists other nursing staff with delivery of nursing care; acts as a team leader/charge nurse as assigned. RN Circulator Positions patients safely, maintains correct anatomical alignment, pads and protects bony prominences Correctly performs skin preps for all procedures accessing for allergies prior to prep and following safety guidelines Carries out medical and surgical asepsis during all procedures. Observes and monitors sterile field for breaks in surgical asepsis. Checks package integrity, expiration dates, and chemical indicator when opening sterile items on to the sterile field. Demonstrates competence in preparing and cleaning of OR suites before and between procedures based on patient’s infection class. Assists in terminal cleaning at end of day. Restocks supplies when necessary and organizes effectively for end-of-case clean up. Assists anesthesia during induction and emergence remaining at the patients side. Demonstrates through knowledge of the inherent risks associated with the use of Electrosurgery units. Practices ESU safety precautions to minimize risk to the patients. Ensures Pre-op Checklist is complete (H&P present, informed consent signed, verification of operative site, patient allergies, NPO status and all pertinent test results and x-rays present) before patient is taken into surgical suite. Consistently introduces self and uses two identifiers prior to interviewing patient for surgery. Confirms correct surgery and site with patient and/ or family. Ensures that surgical site is marked and documents as per hospital policy. Performs "Time out" ensuring correct surgery, correct patient, correct site and side including surgeon, anesthesia and all involved parties as per hospital policy. Appropriately responds to the patient in an emergency or physically distressful situation. Performs and documents counts of needles, sponges, instruments and other countable items as per hospital policy. Effectively communicates counts with the physician and adheres to policy for incorrect counts. Accurately records, handles and labels all specimens according to hospital policy. Demonstrates care of specimens in a manner that minimizes the potential for any errors or specimen mix-up. Takes appropriate measures to maintain thermoregulation in the pediatric to geriatric operative patient. Understands the issues and risk regarding thermoregulation in the per-operative setting. RN Same Day Surgery Coordinates and supervises patient care Performance reflects knowledge in all areas of care specific to outpatient surgery. Gives care to the patient, reflecting flexibility in rules and regulations, as indicated by individual patient needs. Performs a head-to-toe preoperative assessment on all patients and reassesses, as needed postoperatively. This includes pediatric, geriatric and the general patient population. Formulates a teaching plan, based on identified patient learning needs, and evaluates effectiveness of learning; family is included in teaching, as appropriate, from pre-op to discharge. Notifies physician and / or Anesthesia of any abnormal lab/test results. Follows universal protocol for prevention of wrong site, wrong side, wrong person surgery by ensuring the surgery schedules matches surgeon’s orders and H&P. Consistently ensures that the consent is properly filled out & signed before all procedures. Ensures the surgical charts are complete with all necessary documentation, i.e., surgical consent, H&P, labs, EKG’s, etc. Assists Anesthesia providers with blocks and procedures, ensureing the "Time-out" is correctly performed prior to all procedures. Communicates "Hand off" report effectively and thoroughly on the patient’s condition upon transfer from SDS unit to Circulator RN or the receiving unit Registered Nurse. RN PACU Performs head-to-toe pre-procedure assessment on all patients and reassesses intra-procedure and post procedure as needed. This includes pediatric, geriatric and the general patient population. Carries out medical and surgical asepsis during treatments and special procedures. Demonstrates knowledge of perioperative factors that alter immediate post-operative phase. Obtains, documents and assesses vital signs and cardio-respiratory systems every five (5) minutes for changes and provides appropriate treatment or care. Identifies physical symptoms and changes and responds appropriately to the patient in an emergency or physically distressful situation. Communicates with Physician and/or Anesthesia of any pertinent changes in a timely manner. Communicates "Hand off" report effectively and thoroughly on the patient’s condition upon transfer from PACU to the receiving unit Registered Nurse. Maintains moderate sedation competency. Demonstrates knowledge of sedation medications and ability to assess cardio-respiratory systems for changes and provides appropriate treatment. Demonstrates knowledge of airway management and procedures an the ability to use the equipment. RN Charge Additional functions applicable when acting as Charge Nurse: Makes patient care assignments based on the skill, experience, and temperament of other nursing personnel and on the acuity of the patients, ensuring patient, staff safety, assist in the evaluation of performance and skills of Agency/Temporary staff as well as permanent critical care staff. Responsibly directs and precepts staff members as needed/assigned in all aspects of critical care management. Participates in quality and process improvement in the department during all phases from planning to implementing and sustaining efforts.
