Assumes responsibility for patient care and support during on-call hours as scheduled. Qualifications Graduate of an accredited school of nursing. Current Kentucky License. Two years experience as a professional nurse preferred; minimum one year in acute setting. Ability to access patient’s homes independently and cannot have any lifting restrictions. Position Relationships Supervised by Director of Clinical Services. Duties and Responsibilities Coordinates and provides skilled nursing care to individuals and families, based on the existing Plan of Care and nursing assessment of current needs. Functions within the standards of the Nurse Practice Act. Shares coverage of all holidays when Hospice office is closed. These holidays are: New Years Day, Memorial Day, 4 th of July, Labor Day, Thanksgiving Day, and Christmas Day. Explains philosophy, purpose, scope and role of Hospice to patients and families. Completes initial assessment and determines immediate needs of patients admitted during on call hours. Responds to patient/caregiver request for assistance by providing phone instruction, counsel, and support, or by home visit as needed. Provide direct patient care as required, following policies and procedures of Hospice. Provides a safe patient environment, i.e., drug disposal, sharps disposal, and universal precautions. Teaches the patient/caregiver proper use of medications, supplies, and equipment, and the basic skills required to care for the patient in the home. Provides appropriate support at the time of death. Follow Hospice procedure of completing all charting according to hospice policy given and delivering charting to Hospice office in a timely manner. May be required to work in the inpatient unit. Contributes to the overall success of Hospice by cooperating in other organizational requirements and activities. Communicates information concerning work schedule and need for time off to Director of Nursing Service. Participates in Hospice of Lake Cumberland’s orientation and in-service training programs for professional and volunteer staff. Participates in community programs as requested to promote better understanding of Hospice. Participates in organizational committees and special projects as assigned. Attends mandatory in-services and monthly staff meetings. Demonstrates familiarity with Hospice policies and State and Federal regulations governing provision of Hospice care.
