About The Position

Expectations: Responsible for coordination of care to meet identified needs of the patient/family. Supervises the management of patient care utilizing critical thinking and nursing process, discharge planning, resource identification, linking and scheduling patients with required services, monitoring care delivery, advocating for the patient, care planning and evaluating outcomes. Coordinates patient care across the health care continuum, and assists other team members with meeting individual patient needs. This position may require flexibility of hours. EDUCATION: For internal promotions to a RN New Grad or for hires between 9/1/15 – 5/1/2018, must be currently enrolled in a BSN or MSN program & degree must be completed within 3 years of DOH or date of transition to an RN position. Effective for New Hires after 5/1/18, BSN or higher required. Diploma or transcripts on highest degree earned required. EXPERIENCE/TRAINING: Active participation and successful completion of assigned New Grad training program, required. Basic computer skills/experience with computerized documentation systems, required. LICENSES/CERTIFICATIONS: Required: Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN) Current Basic Life Support Provider (BLS) issued by American Heart Association Preferred: Current unrestricted Registered Nurse-Board Certified (RN-BC) or related certification SKILLS: -Ensures all patients' rights are intact. Reports all violations and all patient complaints to Lead RN on unit/shift -Provides therapeutic structure and assist staff therapist in group settings. -Provides patient education. -Continually updates knowledge base in terms of the special needs of the various patient populations served. -Assists in planning and providing care to meet the developmental needs of the patient populations. -Enforces all safety policies and procedures carried out by team members under the supervision of the Lead RN -Follows all applicable guidelines and policies during events such as Code Gray, Seclusion and Restraints, and other Codes. -Cross trained and competent to work on any DMC inpatient unit.

Requirements
