About The Position

The Military to Veterans Affairs (M2VA) Poly-Trauma (PT) Traumatic Brain Injury (TBI) Case Manager Registered Nurse (RN) position is located within the Nursing, Ambulatory Care Service at the Phoenix VA Healthcare System (PVAHCS), Phoenix, AZ. The Military to Veterans Affairs (M2VA) Poly-Trauma (PT) Traumatic Brain Injury (TBI) Case Manager Registered Nurse (RN) executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. Responsible for acquiring and demonstrating clinical expertise and knowledge of health care programs to improve the quality of patient care, the use of resources, and to facilitate compliance with internal and external requirements and standards. Collaborates with other case managers, the interdisciplinary team, and other clinical and administrative staff as needed to ensure patient care needs are met. Provides clinical case management for severely injured and/or ill post 9/11 service members/Veterans with identified case management needs. The M2VA PT TBI Case Managers begin the case management process prior to admission to the facility or evaluation by the interdisciplinary treatment team and work collaboratively with staff from other Veterans Affairs (VA) facilities, Military Treatment Facilities, and referring facilities. Completes an in-depth assessment of functional status, acuity level, prognosis, and assesses the need for treatment services and resources. Initiates contact with the patient and/or family to assist and ensures the transition to the VA is safe and continuous. Monitors the patient's status while receiving care at the facility or during participation in the Individual Rehabilitation and Community Reintegration (IRCR) plan of care. The M2VA PT TBI Case Manager communicates and coordinates with the interdisciplinary team to develop an IRCR-biopsychosocial Comprehensive Plan (CP) of care, for Veterans with poly trauma/traumatic brain injury that are participating in the program. Documents appropriately in the patient record, Care Management Tracking and Referral Application (CMTRA), and the Interagency Comprehensive Plan (ICP). Responsible for coordinating performance improvement initiatives and for collecting data required for accrediting organizations. Exhibits skill and expertise in medical and mental health diagnoses including but not including but not limited to amputation, burns, vision loss, traumatic brain injury (TBI), post-traumatic stress disorder (PTSD), and adjustment/anxiety/mood disorders, as well as insight into the needs of and services required for individuals with these conditions. Serves as an advocate for post 9/11 service members, Veterans with polytrauma or TBI, and their families. Develops and utilizes community resources The M2VA PT TBI Case Manager demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates M2VA PT TBI Program outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented M2VA PT TBI Program outcomes at the program or service level. Program outcomes must be broad and complex and can be demonstrated at any organizational level within a facility or beyond.
