About The Position

The CW RN Manager position serves as the supervisor/lead for the clinical aspects of the Breast and Cervical Health (Women’s Health) program from the CDC. The RN Manager is responsible for supervision of the WH team, work flow, supply management and women’s health screening program including the annual mobile mammography effort. The RN communicates regularly with grant funders designated officials for program guidance, reporting and evaluation. This position provides nurse case management in coordination with current organization needs. A commitment to creating a positive team environment and being a good team member is essential. This position functions within the SEARHC Community Wellness Department, in the Breast and Cervical/Women’s Health Program and is responsible for coordinating the delivery of appropriate and timely care for patients, promoting quality and cost-effective health care outcomes. This position will work with women, ages 21+, by providing comprehensive care management for their breast and cervical cancer prevention and screenings. All while working alongside the patient’s primary care provider’s care team for early detection and screening of breast and cervical cancers, along with determining potential risk factors. Assisting with providing support and resources to women enrolled by assessing their risk factors and social determinants of health. Providing case management for women due for their breast and cervical cancer screenings and women who need follow-up care and diagnostics after an abnormal screening result, along with some care coordination for women diagnosed with breast and cervical cancer. Must utilize sound medical knowledge and clinical judgement in determining patient referrals, outreach, and coordination of care to maintain patient safety and service quality. Decisions must be made with weighted consideration and awareness of the impact that they have on patient care and outcomes. Must be organized, self-motivated and possess the ability to work autonomously to implement care coordination, case management, and identify solutions at an individual, community and system level. Requirements

  • Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required. Direct Hire must have applied for an Alaska nursing license before start date. Agency staff must have an active Alaska license.
  • Graduate of an accredited school of nursing.
  • Basic Life Support certification.
  • High school diploma or equivalent required.
  • Clinical Competency required within 3 months of hire and every three years
  • Manager Competency required within 3 months of hire and every three years
  • At least 1 year of general nursing experience, with at least 6 months employment as a clinical nurse at a SEARHC outpatient clinic OR At least 2 years of general nursing experience with a minimum of 1 year in an ambulatory setting or 1 year case management experience.
  • Knowledge of The nursing process and the ability to apply this knowledge in the working environment.

Responsibilities

  • Responsible for the administrative supervision of nurse, administrative, data, and evaluation staff including orientation, training, and competency oversite.
  • Acts as liaison to Centers for Disease Control leadership to ensure fidelity and compliance with CDC grant funding requirements
  • Supervise and assess appropriate staff competencies as required.
  • Evaluates staff performance and counsels’ staff in performance improvement opportunities.
  • Collaborates with both internal and external partners within the state to promote cancer control and to identify and effectively utilize available community resources to promote health equity.
  • Implement evidence-based-interventions to improve breast and cervical cancer screening rates.
  • Serves as a member of a care team for patients identified for women’s health case management and develops personalized care plans.
  • Advocate patients and patient caregivers at service-delivery level, empower patient decision-making and self-care, and address patient needs in a timely manner.
  • Coordinates care within and outside of the SEARHC healthcare system that is safe, timely, effective, efficient, equitable, and patient-centered.
  • Participate in departmental and clinical improvement efforts, maintain a clean and safe working environment for self, staff, and patients, and maintain compliance with annual competencies.
  • Works closely with Director to ensure that policies and procedures are implemented in the best interest of quality patient care.
  • Communicate effectively with System Director on appropriate departmental issues and concerns.
  • Supervise supply management and inventory in community based and clinical areas.
  • Remains proficient in all applicable computer systems including those for patient care and administrative duties.

  • Works with the team to assure that SEARHC patients are given top priority both personally and professionally by all medical support staff and that the patients and providers’ needs are met through appropriate systems and the highest level of patient service.
  • Support and strengthen working relationships with community-based service providers in rural communities through a coordinated system of care.
  • Promote regular communication and team relationships between community-based providers, clinic staff and case management staff on a local and regional level.
  • Performs other duties as assigned, including special projects deemed necessary to provide comprehensive health services.
  • Assist the Breast and Cervical Health Program in achieving grant requirements and goals.
  • Provide community outreach when necessary.
  • Travel is required.
