Requirements

  • Current unrestricted RN or LPN license in state for which care is being provided.
  • Pediatric experience
  • Certified CPR and negative TB skin test or chest x-ray.
  • Minimum one (1) year of documented experience required in a home health agency, primary care clinic, or health facility, preferred.
  • Possess exceptional nursing assessment skills.
  • Analytic/Critical thinker.
  • Licensed driver with valid automobile insurance.
  • Ability to work on weekends, nights, and holidays.
  • Reliable transportation.
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push, and pull.
  • Varied assistance is required depending on the mobility of the client.
  • Must be able to lift up to 50 pounds

  • Care provided in a client's living environment with varying situations or in an institution such as a hospital.
  • May be required to respond to emergency situations.
  • Travel required.
  • Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.
  • Personal Protective Equipment: May include gloves, mask, eye/face protection, and disposable outer covering or lab coat.
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
  • Varied assistance is required depending on the ability of the client.
  • Must be able to lift up to 50 pounds.
  • Visual and hearing acuity required.
  • Limited travel as needed for various staff and client care service needs.
  • Up to 10% or as directed based on the needs of the business and attending job required training and conferences.
  • Client's Environment: Unknown environments; bodily fluids, household and human germs; extreme cold or hot temperatures in or outside; light to heavy traffic; inclement weather; human body weights that vary based on client health and lifting as prescribed or needed per plan of care; 50+ lbs.
  • All employees are required to pass a background check through Ohio BCI as well as an initial drug screen prior to employment.

Nice To Haves

  • Preferred: Bachelor's degree in nursing.
  • Minimum one (1) year of documented experience required in a home health agency, primary care clinic, or health facility, preferred.

Responsibilities

  • Provides thoughtful care to client and patients based on personalized Plan of Care.
  • Monitors and tracks changes in the client/patient's conditions and needs and keeps the Director of Nursing and any other appropriate personnel informed.
  • Proactively updates client/patients' family on plan of care, health changes, and concerns.
