About The Position

The RN is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit.

Requirements

  • Current and active Florida RN license.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
  • Current and valid Florida driver’s license and automobile insurance.
  • Current CPR certification required.
  • Minimum of one year RN experience where physical assessment skills were utilized.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be able to effectively plan, organize and prioritize work.
  • Must be self-directed and able to work independently in the field.
  • Basic computer skills and EMR software experience preferred.
  • Per Diem works at least one weekend per month.
  • Attendance is a condition of employment.

Nice To Haves

  • Prior experience in oncology, home health, Hospice or palliative care setting is preferred.

Responsibilities

  • Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
  • Clearly understanding and adhering to each Standard.
  • Motivate and lead self with confidence, energy and integrity.
  • Understanding of an ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
  • Perform comprehensive assessments to include the physical, emotional, spiritual and psychosocial needs of the patients and family unit.
  • Establish/update a plan of care based on goals of care as established by the clinical assessment, patient and families’ needs in collaboration with the IDT, patient/family and attending M.D. Update as needed.
  • Provide clinically professional direct patient care to include a variety of skilled nursing tasks to include, but not limited to, assessments, medication administration (PO/SL, Sub cue, IV, PR) wound care, ADLs.
  • Administer medications according to policy and procedures, to include evaluating for response and effectiveness, and document accordingly.
  • Administer medications which include controlled substances, in accordance to physician orders and policy and procedures.
  • Destroy medications that are considered narcotic in nature and all other medication that is no longer being used in accordance with policy.
  • Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients’ plan of care.
  • Complete education requirements in a timely manner.
  • Documentation should include but not limited to variance in patient condition, interventions and outcomes and address progress towards goals set by the patient’s plan of care.
  • Documentation should include education provided to the family and the patient, including topic, which was taught and their understanding of the education.
  • Participate as an active supporter of the Treasure Coast Hospice compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
  • Supervise C.N.A.s and LPNs
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.
