About The Position

Hugh Chatham Health Gastroenterology 201 W. Main Street Elkin, NC 28621 Monday - Thursday 8 am - 5 pm Friday 8 am - 12 pm The following is a summary of the major essential functions of this job. Quality Demonstrates skills and knowledge to effectively complete assigned tasks and completes all required education programs Delivers safe, accurate and timely patient care that conforms to HCMH standards and departmental goals and main job functions as outlined below. Implements an effective internal tracking system for identified patients. Service Assists providers in the clinic with completion of documents for patient care, including screening and scheduling of endoscopy cases Triage phone calls and provide medical advice within the scope of practice and practice protocols Assist with completion of medication refills per office protocols Schedules post operative appointments, anesthesia consults, and history and physical appointments as needed. Assists with scheduling of referrals, other tests and treatments. Liaison for patients for surgical appointments, required tests, insurance verification and all aspects needed to schedule a surgery. Facilitates coordination of care across the care continuum, including: Assists as liaison with patients and their families to physicians, clinical staff, and other departments Acting as a liaison with specialty providers; Proactively acts as a patient advocate, responding to and working to resolve patient concerns; Providing a link to community resources Assists with clinical patient needs as appropriate Performs other duties as assigned. Finance Accomplishes tasks with the minimum expenditure of time, materials, and money with service as the guiding principle. Works productively while on the clock and offers to help others when work is completed. Ensures the accuracy and completeness of clinical information for measuring and reporting physician outcomes. Growth Facilitates patient access to appropriate medical and specialty providers Demonstrates the ability to positively and productively respond to change Facilitates referrals and recommendations for care within the HCMH network while allowing for patient choice and best practices Participates in continuous improvement, offering and or implementing methods to improve care and service

Requirements

  • Graduate of an accredited registered nurse program.
  • Current and valid NC license as a Registered Nurse with biannual renewal.
  • At least 1 year experience in outpatient clinical or community health setting.
  • Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with other team members, physicians, patients and their families.
  • Must have strong critical thinking skills and exceptional ability to integrate knowledge.
  • Excellent observation, communication and decision making abilities.
  • Demonstrates professional and effective written and verbal communication skills.
