Requirements
- Active RN license in the state of Michigan.
- Minimum of one (1) year of nursing experience, preferably in hospice or palliative care.
- Compassionate communicator with strong interpersonal skills.
- Ability to travel within assigned coverage areas; reliable transportation, a valid driver’s license, and auto insurance are required.
- Dedication to providing dignified, patient-centered care.
Responsibilities
- Deliver skilled nursing care and oversee pain and symptom management for hospice patients.
- Collaborate with physicians, chaplains, social workers, and other team members to develop and implement care plans.
- Educate patients and their families on the disease process, hospice philosophy, and available services.
- Provide emotional support and guide patients and families through the end-of-life journey.
- Maintain timely and accurate documentation in compliance with agency, state, and federal regulations.
- Participate in case conferences and on-call duties as needed.
