About The Position

As a Home Health Registered nurse, you’ll provide compassionate care in the comfort of their own homes. You'll be responsible for assessing, planning, and delivering care, all while making sure patients get the best possible treatment in a home health setting. You’ll work independently, using your nursing skills to follow the best practices, while also staying in line with home health guidelines. This role includes utilizing technology, providing therapeutic interventions, and collaborating with other healthcare team members to coordinate care. Requirements

  • Current RN License in the state of practice.
  • Basic Life Support (BLS) Certification for healthcare providers.
  • A valid driver’s license, insured, reliable transportation, and an acceptable driving record.
  • Ability to communicate effectively, both verbally and in writing.
  • RNs with less than 12 months of working experience as an RN must obtain their BSN within 4 years of their start date.

Nice To Haves

  • Bachelor’s Degree in Nursing (BSN)
  • Two years of experience as an RN.
  • Experience in Acute Care, Home Health, or Hospice settings is a plus.

Responsibilities

  • Provide nursing assessments, interventions, and evaluations for adult and pediatric patients in their home setting.
  • Develop personalized care plans to meet the physical, emotional, and psychological needs of patients.
  • Use the nursing process to ensure care is delivered effectively, efficiently, and in compliance with policies and regulations.
  • Provide education and support to patients and their families to help them understand and manage their health conditions.
  • Collaborate with other members of the healthcare team to coordinate and deliver care.
  • Document all care provided in compliance with home health guidelines.
  • Ensure a safe and healing environment for patients, helping them feel supported and cared for in their home.
