About The Position

Registered Nurse - Homecare Visiting Description We, OM A MA Service Incorporated, are searching for a Registered Nurse to be part of our growing home health care organization as a contracted visiting nurse. Your responsibilities include but are not limited to: Support and sustain OM A MA 's commitment to compliance. Managing caseload to ensure quality and safe delivery of nursing services to patients in the home setting. Day shift home visits, but exact scheduling is flexible depending on case. Develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care. They provide coordination of the plan of care, maintaining continuity of the client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services. Supervise the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conduct supervisory visits per regulatory guidelines. Providing quality and safe delivery of home health care services. Record and document patient care services. Promoting education for clients, client families and the community that reflect VNCC’s philosophy and standards of home health for assigned clients.

Requirements

  • Current RN licensure in MA (RI is a plus).
  • 1--2 years med/surg, or acute care experience preferred.
  • 1--2 years Home care experience preferred.
  • CPR/AED certification.
  • Valid driver’s license and available transportation.
  • Strong assessment and communication skills.
  • Ability to build rapport with patients.
