About The Position

The Veterans Integrated Services Network (VISN) Veterans Affairs (VA) Home Telehealth Care Coordinator Registered Nurse (RN) coordinates care for a panel of patients throughout the continuum of care with a focus on coordinating the appropriate intensity of care for the home telehealth patient panel. The Registered Nurse (RN) Home Telehealth Care Coordinator is responsible and accountable for individual nursing practice and determines the delegation of tasks consistent with the nurse's obligation to provide optimal care. Manages assigned panels, utilizes registries, and ensures appropriate evaluation and access is provided to patient panels regarding telehealth health promotion and disease prevention. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. Coordinates appropriate intensity of care management for the home telehealth patient panel with a focus on providing more coordinated and higher quality care to complex clinical cases with a goal to reducing hospitalizations and ED visits and cost while providing increased access for patients.

Requirements

  • Licensure: 1 full and unrestricted license from any US State or territory

Nice To Haves

  • One to two years of experience in telehealth or home telehealth nursing, with demostrated proficiency in remote patient monitoring, care coordination, and the use of virtual health technologies.

  • Knowledgeable of ethical issues related to professional nursing practice and follows established policies of the practice setting, VA, and ANA Code of Ethics for Nurses.
  • Aware of high reliability principles to deliver consistent care and improve patient outcomes.
  • Demonstrates positive, effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers.
  • Participates in unit based educational activities and continuing education requirements.
  • Responsible for maintaining competency to continue personal and professional growth.
  • Applies evidence-based practice/research to patient care.
  • Participates in unit-based activities to improve and deliver cost effective patient care.
  • Incorporates patient preferences into shared care delivery decisions.
  • Works with close supervision, is responsible and accountable for individual nursing practice and seeks direction from others as needed.
  • Manages workload as assigned, organizes and completes own assignments in an efficient and appropriate manner.
  • Participates in the development, implementation, and evaluation of interdisciplinary care.
  • Additional duties as assigned.
