Requirements
- Graduate of an approved nursing program (BSN preferred).
- Current RN license in the state of practice.
- At least 1 year of professional nursing experience (Home health experience is a plus!).
- Strong computer and technology skills.
- A valid driver’s license and reliable transportation.
- Commitment to our mission, ethics, and compliance standards.

Responsibilities
- Conduct comprehensive assessments of patients’ health status and needs
- Develop and implement individualized care plans in collaboration with interdisciplinary teams
- Administer medications and treatments, monitor patient responses
- Educate patients and families on disease management and health maintenance
- Monitor vital signs and manage chronic conditions
- Accurately document patient care activities
- Coordinate care with physicians, therapists, and other healthcare professionals
- Provide emotional support and counseling to patients and families
- Ensure compliance with healthcare regulations and standards
- Participate in quality improvement initiatives and ongoing education
