About The Position

The Iowa City Home and Community Based Services (HCBS) Program assists in providing options to Veterans on how they would like to receive care with the intention of providing the right care, in the right place, at the right time. The HCBS team is focused on Aging in Place and the 4Ms (What Matters, Medication, Mentation and Mobility) adapted by the Age-Friendly Health System Movement and encouraging restorative care versus dependence. The Registered Nurse-Home and Community Based Service Case Management works as part of an interdisciplinary leadership team guiding this care across the facility. The case manager is responsible for coordinating the care of Veterans receiving both internal VA and external community care-based care within any of the Home and Community Based Services including, but not limited to, skilled home care, non-skilled home care, adult day health care, and respite care. This involves ensuring there is not duplication of services between internal VA programs and community-based care.

Requirements

  • 1 full and unrestricted license from any US State or territory

Responsibilities

  • Collaborates with the VISN 23 HCBS team to develop policy/procedures to support Veterans and those who care for Veterans.
  • Provides leadership in establishing, coordinating, and integrating accessible quality healthcare services for Veterans requiring care within multiple healthcare delivery systems throughout the health care system.
  • Performs program analysis, development, review, research, design, and contact work with various agencies (public, DOD, state, etc.) as needed.
  • Administers information and analytical systems to evaluate and enhance the quality of service provided to patients.
  • Develop, implement, and evaluate clinical guidelines and protocols to ensure appropriate utilization of services.
  • Conducts and analyzes studies and projects fundamental to planning, programming, and budgeting of Iowa City HCBS such as forecasts of technological changes, bed utilization, future patient workloads, staffing projection and cost comparisons providing feedback and recommendations to Iowa City Leadership that reflect current standards and expectations provided by VISN 23 and VA Central Office.
  • Mentors and supports Iowa City staff on HCBS associated national policy and guidelines.
