About The Position

St. Mary Medical Center, a member of Trinity Health Mid-Atlantic, is seeking an experienced RN Case Manager to join our team! The Case Manager is responsible for coordinating and facilitating all aspects of discharge planning needs of assigned patient population. Modifies approach based on age specific needs of patient population.

Requirements

• Bachelor's degree in nursing required.
• Active PA RN License required.
• Previous experience in Case Management highly preferred.
• Demonstrates ability to use problem solving, critical thinking, and priority setting skills.
• Must be able to work independently and on a team.

Nice To Haves

• CCM or Accredited Case Manager certification preferred.
• Previous experience in Case Management highly preferred.
