Purpose: provides comprehensive, patient-centered care, focusing on health promotion, chronic disease management, and preventative services. Key responsibilities include patient assessment, care coordination, medication management, and patient education. Works closely with physicians to manage patient health, optimize outcomes, and provide telehealth advice, ensuring continuity of care. Requirements

  • License: Current Registered Nurse (RN) license in the state of Florida
  • Education: ADN or BSN from an accredited nursing program.
  • Experience: Previous experience in primary care or family medicine is preferred.
  • Certifications: BLS (Basic Life Support) certification from the America Heart Association.
  • Skills: Strong communication, clinical assessment, and team collaboration skills.

Responsibilities

  • Clinical Assessment: Conducts physical examinations, records vital signs, and takes patient histories.
  • Chronic Disease Management: Manages conditions like diabetes and hypertension, adjusting plans based on provider orders.
  • Care Coordination: Coordinates care across settings, including managing hospital transitions and referrals.
  • Medication & Procedures: Administers medications, vaccinations, and performs wound care, IV access, and diagnostic tests (e.g., EKG).
  • Patient Education: Educates patients on lifestyle modifications, disease prevention, and treatment plans.
  • Triage: Provides telephonic advice and urgent, in-office triage to prioritize patient needs.
  • Documentation: Maintains accurate, HIPAA-compliant records in the Electronic Health Record (EHR - Epic)
