About The Position

The Western New York VA Medical Center is seeking a Registered Nurse (RN) - Assistant Program Director to work on the Buffalo Home Based Primary Care (HBPC) Team. This RN is located at the Buffalo facility. This Registered Nurse provides direct Veteran care and care coordination, to Veterans and their care within the HBPC Program, and is responsible for case management, assessment, triage, Veteran education, resource coordination, medication/treatment coordination, and care planning. The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) Program Assistant Director executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex home based primary care. This RN provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program, and are responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. This incumbent will plan and deliver technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, palliative care, and utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. The HBPC RN does home visits to Veterans in the program, collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines, and will demonstrate performance and leadership that is broad enough to improve the care for the home based primary care program and facility.

Responsibilities

  • Make home visits to Veterans in the program and assess, plan, implement, and evaluate care based on age specific components.
  • Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Influence care outcomes by collaborating with members of the interdisciplinary team.
  • Support colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care, and foster a safe and supportive environment conducive to the professional development of health care professionals.
  • Evaluate outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
  • Responsible for the documented HBPC outcomes at the program or service level.
  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
